What Is The Document Called The “Statement of Issues”?

If you are applying for a type of medical license in the State of California and your application is denied, you will receive a document known as the “Statement of Issues”. The “Statement of Issues” provides the legal basis for denying your application.

The “Statement of Issues” will usually consist of the following sections: 1) Parties; 2) Jurisdiction; 3) Statutory Provisions; 4) Regulatory Provisions; 5) Factual Allegations; 6) Causes for Denial; and, 7) Prayer.
